It’s a simple 3-step process to be certified for a captioned phone at no cost.
Step 1
Download the printable PDF form.
Step 2
Complete your information and forward to your audiologist or other certifying professional to provide certification information and a signature.
Step 3
Your certifier can submit the form to Hamilton CapTel on your behalf or forward back to you if you prefer to do so (submission information is on the form). You’ll be contacted within a few days by a Hamilton CapTel representative to verify delivery and/or schedule installation assistance if needed (please allow 1-3 weeks for delivery).
